Immunization & Health Documentation

Before entering Colorado College, all new students are required to submit documentation about a number of immunizations and other health screenings (please read the entire page, as COVID documentation is submitted through a different portal than other required documentation).  This is the only way we will be able to track and accept documents that are submitted.  

Each of the required forms is available to open and print below.

The first 2 pieces of documentation (your Colorado Certificate of Immunization and Tuberculosis Screening Questionnaire forms) through the Admission applicant portal:

  1. The Colorado Certificate of Immunization (required by the state of Colorado), a 2-page document that demonstrates proof of:
    1. MMR (measles, mumps, and rubella) immunization OR a signed statement of immunization exemption based on medical, religious, or personal reasons (separate documents; see below). Please note: you may be asked to leave campus in the event of an outbreak, should you waive the immunizations.
      • Medical Exemption Form must be completed and signed by a medical provider.
      • Non-medical Exemption Form must be completed and signed by the student and/or parent/guardian. This form also REQUIRES either the signature of a medical provider OR a certificate of completion of the Colorado state Online Immunization Education Module.  You can access this education module here.
    2. Meningococcal disease immunization OR a signed acknowledgment that you have reviewed the information and decided not to obtain the immunization (Page 2 of the Colorado Certificate of Immunization). Please note: you may be asked to leave campus in the event of an outbreak, should you waive the immunizations.   2. Tuberculosis (TB) Screening Questionnaire completed within the 6 months prior to arrival on campus with the signature of your health care provider.
  3. Additionally, all students are also required to submit COVID-19 Immunization Documentation (please see below for instructions on submitting this documentation): 
    • CDC or WHO COVID-19 Vaccination Card, which can be submitted through the COVID-19 Vaccination Documentation Portal.  Please note that all Colorado College students, staff, and faculty who are eligible are required to get a COVID-19 vaccine booster by Jan. 3, 2022. If not yet eligible, you must receive your booster as soon as you are eligible. If you submitted your initial COVID-19 Vaccination documentation in the Fall of 2021, please click the portal link above and follow these instructions to edit and submit your booster shot documentation. 
      • You will see a message that says “Sorry. You’re already registered for this form.” Please click the Home link in the top-left-hand corner of the screen.
      • At the top of the page, click on the red box that says "Actions Required."  
      • In the Answer Additional Questions section, click on the "Answer Questions" tab.
      • If you are not taken directly to the “Student COVID-19 Vaccination Documentation” form, click on that in your menu, and click Next at the bottom of the page.
      • Scroll to the bottom (scroll past the already answered questions) to the question "Have you received a booster vaccination?"  and click the Yes button to indicate that you have received a booster.
      • On the next page, upload your COVID vaccination card showing the date of your booster.
      • Please answer the questions about the date and type of booster vaccine received.
      • Scroll down to the bottom and answer No to the question about your second booster.
      • Click Save and Continue.  (you will not receive any kind of confirmation for the booster upload, but if you follow these steps your booster information will be captured.)
    • OR a signed statement of immunization exemption based on medical, religious, or personal reasons (separate documents; see below).  Please note: you may be required to go into quarantine if exposed to COVID19, or asked to leave campus in the event of an outbreak, should you waive the immunizations.
      • COVID-19 Medical Exemption Form must be completed and signed by a medical provider and can be submitted through the COVID-19 Exemption Request Portal.
      • COVID-19 Non-medical Exemption Form must be completed and signed by the student and/or parent/guardian and can be submitted through the COVID-19 Exemption Request PortalThis form also REQUIRES a certificate of completion of the Colorado state Online Immunization Education Module.  If you are submitting a non-medical exemption for MMR, you can submit the same certificate of completion of the education module for both MMR and COVID-19.
        • The Colorado state Online Immunization Education Module is available 24 hours a day and seven days a week. It can take approximately 20 minutes to complete. In order to successfully move through this Online Module and obtain the Certificate, you must review the Technical Guidance and Troubleshooting Document.”  At the completion of the module, you will be able to complete an electronic form to obtain a Certificate of nonmedical exemption. You can complete this form for more than one child without having to retake the course. Access the education module.
