You must use a consent form unless you have submitted the request for a waiver or alteration of the consent process and have had your request approved by the IRB. You must use one of the below templates unless the IRB has approved your using a different consent form.

While the IRB is working remotely, please remove Amanda Udis-Kessler's phone number and mailing address from all consent, assent, and parental permission forms, leaving her email in place.

• Request for Waiver or Alteration of Informed Consent Process - submit if you are requesting a waiver or alteration of the informed consent process, or if you are requesting a waiver of documentation of informed consent
