All forms can be submitted either in paper format to the IDM office (230 Suite in Tutt Library) or as a scanned document emailed with signatures to astoller@coloradocollege.edu.

IDM application

To submit an application, all four parts of the application (including faculty letters) are due to the OAP Director no later than 5pm on the dates posted on the "How To Apply" section of this website. The last date to submit an application is block 7 of a student's sophomore year.
