Completing the Annual Evaluation
Please retrieve the 2022-23 the performance evaluation form located on the HR website and complete your self-review. You can reference this document to help you write your reflection, learn about goal setting theory to help you look ahead at priorities for next year, and review these examples for ideas about how you can ask your supervisor for the support you need.
After you finish your self-review, send the form to your immediate supervisor so they may add their comments.
Additional items that supervisors and staff may want to review in preparation for the performance evaluation are the job description, last year’s performance evaluation, and any additional goals that have been identified during the review period.
Plan ahead by having notes on your employee’s performance during the review period and by identifying work priorities for the upcoming year ahead of time. Once you receive your employee’s self-review, read their comments and add your comments in all sections to prepare for the performance review meeting.
Because performance evaluations can create sense of stress or anxiety for everyone involved, make a plan to send the completed review to your employee prior to the scheduled meeting. The goal of the annual evaluation is to have a meaningful discussion about the employee’s performance and ways in which you can support their development. If you get stuck, here are some conversation starters you can use to help you make this a good experience for you and your staff member.

- All staff members complete the Staff Annual Performance Evaluation form as their self-review.
- Supervisors will receive the completed self-review, add their comments in all sections and add (or edit) the priorities and goals for the upcoming year.
- Supervisors complete the "Performance Summary" statement that best describes the staff member's overall performance by checking the appropriate box. Refer to this document for definitions.

- Successfully Meets Expectations
- Does NOT Meet Expectations
- If the supervisor rates an employee ‘Does NOT Meet Expectations’, the supervisor is to communicate this information with their Department Head and Laurie Mozingo ahead of the formal meeting with their employee.
- Supervisors send the form back to the employee prior to the formal performance evaluation meeting.
- During the meeting, the supervisor and employee meet to review the completed form and discuss the goals/priorities for the upcoming year. This is also the time to discuss any support needed from the supervisor to ensure the employee can perform effectively and be successful.
- After discussion, the supervisor finalizes the document and both parties will sign and date the form.
- Supervisors email the fully executed document to CC Performance and to the employee for their records.
