  3. If not in database, ask patron to complete a General Application card
  4. To enter new patron, click NEW icon and select the “Community Borrower” template.
  5. Use tomorrow and the next year for expiration date. Then click NEXT.
  6. Enter Patron name: Last, First, Initial (i.e. Doe, John, M). Then click NEXT.
  7. Address box: First line is for street (1021 N. Cascade Avenue), second is for city, state, zip (Colorado Springs, CO 80903). Then click NEXT.
  8. Telephone: Obtain both home and cell, if appropriate. Then click NEXT.
  9. Patron barcode: get barcode from “Forms Shelf” and put on patron library or ID card; scan the barcode. (If the barcode is a duplicate, then close the window you are in, finish with card and ask a supervisor for help). Then click NEXT.
  10. For the Message Box use the Initials of the Public Library they are from (i.e. PPLDSPLDPLMSPL, etc). Then click NEXT.

  12. Fill in “Notice Preference” (phone, email, print)
  13. Ask patron for the name of the organization/affiliation who will be using the music. Add this information in an “x-note” (see the drop-down box in the “Insert” options). Precede name of organization with “SYL” – for example, “SYL-Pikes Peak Philharmonic.”
  14. On the back of the application card, note “SYL” as we do for SMF.
  15. SAVE the card.

All Symphony Library users must initially complete a “Symphony Library Rental Agreement” before checking out SYL items. This agreement will be kept on file as a permanent record and will apply to all future checkouts. These forms and signed agreements are kept in a binder under the Circ Desk labeled “Chamber Orchestra/Symphony Library Procedures.”

A fee of $20 per each request (up to 5 selections) is also required.
