GENERAL INFORMATION

BOETTCHER HEALTH CENTER MISSION STATEMENT

Boettcher Health Center strives to foster an environment that supports the educational mission of the Colorado College. The Health Center provides students with urgent medical and mental health care for acute illness, health and wellness education to prevent future illness, and epidemic control for the entire college community. Boettcher Health Center also strives to promote responsibility in students for their health care, create informed medical consumers, and remove any physical and mental health related barriers to the academic and extra-curricular success of each student. Health is more than the absence of disease and the Health center strives to promote physical, mental, and social well being in the Colorado College student.


Confidentiality and Consent for Treatment

All medical records are confidential by law, unless you give us your written permission to release your records to your parents at the time of your treatment. We notify your parents only if you are in a life-threatening emergency. According to Colorado state law, if you are at least 18 years old or married, you have the right to authorize any medical treatment without your parents' consent. If you should require ambulance transport and/or hospitalization and are unable to give your verbal consent for treatment, the medical staff at Boettcher Health Center will, by virtue of your enrollment at Colorado College, have your implied consent to direct your treatment and to inform your parent(s)/guardian and the college administrator on emergency call duty of your medical situation. Charges posted to your student account are kept confidential by using a code.

Medical Records

Each student has an individual medical record at Boettcher Health Center. All medical records and the information contained within them are strictly confidential. Policies and procedures that comply with the HIPAA regulations for Standards of Privacy and Security have been implemented and are strictly followed. Patients may access, review, request copies, and/or request changes/amendments to his/her health records. Patients requesting access to his/her medical information will be asked to complete a REQUEST TO ACCESS PROTECTED HEALTH INFORMATION (PHI) form (available on this site under forms). Upon receiving the written request from the patient, an appointment will be made within a reasonable time, and when a health care provider can be available. Patients requesting copies of any portion of his/her healthcare record must complete the AUTHORIZATION TO RELEASE MEDICAL INFORMATION form (available on this site under forms) in its entirety. Depending on the number of copies, the patient may be asked to return at a later time to retrieve the copies. The one exception that does not require the AUTHORIZATION TO RELEASE MEDICAL INFORMATION is the immunization record(s). Patients may ask that immunization records be mailed, emailed, or faxed to himself/herself, or to the party of their choice (e.g., another college admission office or healthcare center, healthcare provider, employer, etc).

Charges for processing, copying and/or faxing medical records are as follows:

  1. first five pages are no charge;
  2. $10.00 for the next six to ten pages, and;
  3. $.50 for each additional page.

Records that exceed ten pages will not be transmitted by fax machine, rather will only be mailed to the party the records are being released to. Postage costs will also be charged. All documents that are transmitted by fax machine will be transmitted with a FAX cover page. BHC retains medical records for 10 years after a student graduates or withdraws from CC. All out dated records are shred as the means of disposal.

Health Records Required For New Students

Because of the close living proximity in our residential halls and to ensure students are healthy, we require that all new students have a physical exam prior to arriving on campus. The Student Health Record and the Colorado Certificate of Immunization for College Students forms are to be completed and returned to Boettcher Health Center before students arrive on campus. These forms are located on the Health Center’s website at http://www.coloradocollege.edu/students/current/boettcher/pdfs/. Using the forms provided by Colorado College will assist students, parents, and doctor offices in submitting complete health records. To allow the College’s medical staff sufficient time to review and develop your patient record, please note the following deadlines for the 2009-10 academic year to return your physical exam and immunization records:

First-year or transfer students who register in August: August 14, 2009
Winter start or transfer students who register in January: December 11, 2009

Some of the frequently asked questions that we are get from new students and parents are:

Does the student have to have a physical exam?
Yes, because of the close living proximity in our residential halls and to ensure students arrive healthy, we require that all new students have a physical exam prior to arriving on campus. Please check to make sure your provider signs the physical exam form..

Is the physical for athletes the same as the physical exam for new students?
No, the physical exam as a new student is much more comprehensive than the physical exam performed by Boettcher’s staff for athletes. The physical exam for athletes emphasizes different areas, including more orthopedic data, than the physical exam for new students.

Can I have my physical done at Boettcher Health Center when I arrive on campus?
No. Your physical should be completed before arriving on campus. The Health Center is not staffed to do physicals for the number of new students arriving. As well, the activities of the new student orientation week complicate scheduling a new student physical.

Are there specific forms that I should be using?
Preferably yes. The forms are located in the CC Bound notebook under the Student Services section, Boettcher Health Center. The physical exam/health record and immunization forms are also located on the Health Center’s website at http://www.coloradocollege.edu/students/current/boettcher/pdfs/ . Using the forms provided will assist doctor offices and students to submit complete health records.

Why are a hematocrit/hemoglobin and a urinalysis test required?
A hematocrit/hemoglobin test screens for anemia, while the urinalysis test screens for diabetes and possible bladder infections. Having these test results on file provides a baseline result to compare with if the student becomes ill.

Is a TB (PPD) test required?
Yes. Tuberculosis (TB) is a disease caused by bacteria called Mycobacterium tuberculosis. The bacteria usually attack the lungs, but, TB can attack any part of the body such as the kidney, spine, and brain. If not treated properly, TB disease can be fatal. CC has students from all over the world, some from areas where TB is still common. Students may be in close contact with a student who has been exposed to TB; the PPD test is for their protection. A TB (PPD) test must have been administered within six months and prior to registration. The results of the TB test must be documented or the TB test is not complete.

Why do you need my health history?
Complete information on your health history is vital for the Boettcher staff to diagnose and treat your illness. To ensure continuity of care, it is advisable that all sections pertaining to previous or current health care conditions are noted. Additional medical history may be noted on the back of the form.

What immunizations are required?
It is a Colorado law requirement that all students have two doses of “MMR” (measles, mumps, rubella) vaccine given after the age of 12 months. The vaccine date must include the month, day, and year. In the event of a measles outbreak, any student who has not had two doses of measles (rubella) vaccine will be required to leave campus. While the meningitis vaccine is not required, it is highly recommended. If you have not received a meningitis vaccine prior to arriving at Colorado College, please complete the back of the immunization form which is not a waiver but indicates you are aware of the risk of meningitis. The meningitis vaccine is available at Boettcher Health Center. Colorado rules, regulations, and laws pertaining to immunization requirements for college students can be viewed under Section 15 at http://www.cdphe.state.co.us/dc/Immunization/immunmanual/immunmanual.html.

If I’m not 18 year of age by the time I arrive on campus, do you need my parent’s permission before the Health Center can provide treatment?
Yes. Parent(s)/guardian(s) should carefully review the AUTHORIZATION FOR MEDICAL CARE OF A MINOR (located on Boettcher’s website), select the option that fits your situation, and sign the form. Once the student turns 18 years of age, this form is removed from the patient record.

Should I bring my forms to campus when I arrive?
Preferably, no. All records received prior to the student arriving on campus will expedite the registration process. We do understand though that some students are not able to complete their physical exams and TB tests until just before coming to campus.

What happens if my forms are not complete?
The health forms are reviewed immediately when received. Forms that are missing required health information are returned to the student with a request to complete the missing information. Students with missing requirements of their health records will not be able to complete the registration process until the records are complete.

Student Health Insurance

All degree-seeking students taking credit hours and all Cultural Program Coordinators are required to have health insurance as a condition of enrollment.

Good health is essential for students to obtain the most benefit from participation in college life. It is not uncommon for students who attend college to experience more illness and injury than they do at home. Colorado College recognizes this and desires to safeguard each student’s health and financial protection. For students who do not have coverage through a family plan, or for those who have insurance plans that do not offer sufficient coverage, we are pleased to sponsor an excellent health insurance program underwritten by United Healthcare Insurance Company. To compliment and supplement this tremendous value in financial protection, Colorado College operates a health care and counseling center during the academic year with fees and charges that are substantially less than those charged in the community. All currently enrolled students are welcome and encouraged to use Boettcher Health Center, whether or not they are enrolled in the student health insurance plan offered through United Healthcare.

We suggest that you review your current health insurance plan for allowable benefits in the Colorado Springs area to make sure that there is no geographic location, limited provider network, specific illness, or other restrictions, such as referrals to off campus specialists by Boettcher Health Center providers. Although students with other health insurance may waive participation in the student health insurance plan, we encourage you to consider the exceptional value of this comprehensive program offered through United Healthcare. We have found that the student insurance policy pays on average 92% of the claims filed, while private family insurance policies pay on average only 8% of the claims filed.

Eligibility
All degree-seeking students taking credit hours and students on an approved study abroad program are eligible to enroll. Cultural Program Coordinators are required to purchase this insurance plan. Students must actively attend classes for at least 31 days after the date for which coverage is purchased. United Healthcare maintains the right to investigate student status and attendance records to verify that the policy eligibility requirements have been met.

Enrollment periods and rates:
The student insurance plan offered through United Healthcare Student Resources becomes effective on August 1, 2009 and terminates on July 31, 2010.

How to waive student insurance

All students will be automatically enrolled and charged for student health insurance premium of $1720 unless an Online Waiver form is filled out and a front and back copy (must be legible) of the student’s health insurance card is received by Boettcher Health Center’s student insurance office. To waive the student insurance plan go to www.srstudentcenter.com and follow the waiver instructions. The deadline for filling out your waiver form is September 18, 2009. Students who have not waived by that time will not be eligible for a refund of the insurance premium.

Covered medical expenses at Boettcher Health Center include HPV vaccine, STD testing is covered up to $150 maximum, acupuncture benefit is provided at BHC only, up to 12 visits maximum.

Students are encouraged to use the resources of Boettcher Health Center first because our fees are substantially less than providers in the community and many services are covered at 100%. An added benefit to the substantial cost savings is our convenient location on campus. Students may need to use outside providers when Boettcher Health Center is closed and are encouraged to select a network provider. Students choosing to use outside providers, without referral by a BHC provider, will have benefits paid at a substantially reduced rate. The referral requirement is waived if Boettcher Health Center is closed or the student is 50 miles or more from campus.

Intercollegiate sports coverage
Athletes who enroll in the Intercollegiate Sports Rider option must also be enrolled in the student health insurance plan through United Healthcare. If you have questions regarding coverage or enrollment, please contact the athletic insurance coordinator, Celina Swedlund at (719) 389-6321

Resources
United Healthcare insured students have online access to claims status, Explanation of Benefits, correspondence and coverage information via My Account at www.uhcsr.com. Insured students can also print a temporary ID card, request a replacement ID card and locate network providers. Colorado College’s policy number is 2009-1025-1. Insured students have access to nurse advice and health insurance information 24 hours a day, 7 days a week by dialing 1-877-950-5004. The Collegiate Assistance Program is staffed by Registered Nurses who can help students determine if they need to seek medical care, understand their medications or medical procedures, or learn ways to stay healthy.

Coordination of Benefits
A coordination of benefits will occur with any other group medical, surgical or hospital plan so that combined payments under all programs will not exceed 100% of the charges incurred for covered services and supplies. The student health insurance plan is always primary.

Master Policy
The information on this website is only a general summary. The Master Policy is on file at the College and contains all of the provisions, limitations, exclusions and qualifications of the insurance benefits, some of which may not be included in the summary. The Master Policy is the contract that governs and controls payment of benefits.

How do I submit charges from Boettcher to my family insurance?

Students are given an itemized insurance claim form with all of the coding necessary for your insurance company to process your claim. The claim forms are sent to your Worner mailbox. It is then up to you to forward the claim forms to your insurance company, or home to your parents to submit them to your insurance company. Due to our strict confidentiality practices, the itemized statements for insurance claims will be released only to the students, and not to parents. Payments received from your insurance company will be posted to your student business account.


How do I make an appointment?

Valid medical emergencies are seen immediately. Patients late for appointments may have to forfeit their time slot and reschedule. We respectfully request a 2 hour notice if the patient does not intend to keep an appointment. Missing appointments that have not been cancelled deprives other students of getting health care. You will be charged the cost of the appointment if you are a “no-show”.

Emergency Room and Hospital Care

Hospital emergency rooms should only be used for life threatening emergencies or if special treatment is needed. In the case of a serious emergency, an ambulance should be called. The cost of ambulance transportation is the student's responsibility to pay. For minor emergencies, other students, residence hall staff, or Campus Safety can be asked to bring students to Boettcher Health Center. There are two major hospitals in Colorado Springs. Penrose/St. Francis Hospital is approximately 10 blocks from campus and Memorial Hospital is approximately two miles from campus. Because of the close proximity, Boettcher Health Center staff will typically refer students to Penrose/St. Francis Hospital. Students and parents should notify Health Center staff if Memorial Hospital is the preferred net-work hospital for their family insurance plan.


Physicals/Medical Screening

Students are often required to have physicals for study abroad programs, employment, the Peace Corps, or for outdoor programs (Outward Bound, NOLS). The physician or a mid-level provider at Boettcher Health Center can complete these physicals.

Following are the guidelines used when students call or come to the window requesting a physical:

  1. First, ask if the program they need the physical exam for is a “CC-program” or a program sponsored by an organization off-campus (i.e., SIT, ACM, NOLS, Peace Corp). Physicals for Colorado College programs do not require lab work before the forms can be completed. Some of the off-campus program physicals require specific lab work and documentation of immunizations.
  2. Physical exam appointments should be given a 30 minute time allotment. Students must bring completed forms to their appointment, or the appointment will be rescheduled. Boettcher Health Center does not have a supply of the forms; the student must get the forms from their professor, the Summer Session Office or the Office of Study Abroad and International Students.
  3. Because of the length of time set aside for a PE, no more than three physical exam appointments per provider per day will be scheduled.
  4. The student should be referred to Boettcher’s Immunization Clinic to schedule an appointment to review recommended immunizations.
  5. If a pap smear is required, a separate appointment should be scheduled with the GYN-NP.
  6. The physical exam required for new students must be performed by a family physician or other health care provider, and prior to the student arriving on campus.

CC PROGRAM – Schedule a 30 minute appointment with a provider taking care not to schedule more than three physical exams per provider per day. Physical exams for off-campus programs sponsored by Colorado College are administered by Boettcher Health Center (BHC); call 719-389-6384 to schedule an appointment. Bring all parts of this form to your appointment. Students are encouraged to allow sufficient time to schedule an appointment in order to meet the deadline date set by the course instructor. A copy of the physical form will be kept on file at Boettcher Health Center.

ALL OTHER PROGRAMS:
Ask the student to bring the health forms to the Health Center prior to scheduling an appointment. A nurse or the lab tech should look at the forms to determine if lab work is required prior to completing the physical forms.

If no lab work is required:
Schedule a 30 minute appointment with a provider taking care not to schedule more than three physical exams per provider per day.

If lab work is required:
Lab hours are Monday through Friday at 8:15 a.m. to 2:30 p.m. Students are required to bring the forms or instructions for the physical they are to receive with them. (Examples of the usual types of lab work requested is a hematocrit/hemoglobin (blood test that screens for anemia), urinalysis, and a TB (PPD) test, but some programs require other specialized testing such as HIV, blood type, etc.) Upon completion of the lab work and TB test, the student is referred to the front desk to schedule an appointment to finish the physical examination. If the physical cannot be scheduled within the two to three day time period to read the results of the TB test, the student will be asked to return to have the TB test interpreted, and the physical scheduled at the next available time. Schedule a 30 minute appointment with a provider.

VISA Medical Statement
In order to obtain a VISA, some off campus programs require a medical clearance statement. The form must be printed on Colorado College Boettcher Health Center letterhead and be signed by a physician (Dr. Reynolds). Because Dr. Reynolds’ is not in the clinic on a daily basis, the form may be left for her signature and the student can pick it up at a later date.



Women's Health

A certified gynecological nurse practitioner is available for:
  • Routine gynecological exams
  • Pap smears
  • Birth Control Methods including discounted birth control pills
  • STI testing
  • Counseling related to reproductive health, sexuality, and relationships

Please feel free to make an appointment for any of these services. You and your partner are welcome to come in and discuss any of these issues.

Men's Health

Men's health problems and concerns are treated confidentially and respectfully at Boettcher Health Center. Health care providers are trained and experienced in providing STI (sexually transmitted infections) testing and evaluation (including urine and blood tests, HIV counseling and testing, instruction in testicular self-exam) and are willing to discuss male sexuality issues. Condoms are available 24 hours/day at the clinic for a discounted price. A variety of educational publications are available, reviewing abstinence, birth control, STI's, and sexuality concerns. Men are welcome to bring their partners to discuss pertinent issues during appointment hours.

The Counseling Center in Boettcher Health Center

The Colorado College Counseling Center is a confidential place where students can receive psychological consultation, short-term counseling, and information on a variety of issues. Mental health counselors are on-call for crisis intervention and can be contacted through the Health Center. Some students seek counseling for concerns which arise while they are at college. Students also seek out counselors to work on issues which started before their arrival at college, and continue to be troubling to them. These concerns include difficulty with academics, depression, and problems in their families and intimate relationships. The Counseling Center staff is a group of professionals who have a number of office hours each week at Boettcher Health Center. Each school year, hundreds of students seek out the assistance of the counselors.

Our individual counseling and mental health services are confidential and short-term. We can focus on assisting students with personal, social, and academic concerns, including: study problems, adjusting to a new environment or culture, anxiety, decision-making, loneliness, depression, body image, anger, grief, personal crisis, and relationship issues. The counseling staff is available to consult with anyone who has questions about their own issues or to answer questions and provide options about other situations causing concern. We are happy to provide referrals to off campus resources and provide options about treatment alternatives. We encourage individual students and student organizations or groups to call on our staff to offer workshops, discussions, or groups on a wide variety of topics.

  Psychiatrist
The consulting psychiatrist at the Counseling Center can work with students to manage any medications which might be necessary. Students can also be followed for medications they were prescribed by their doctors at home.

  Scheduling an appointment
We encourage students to schedule an appointment by calling extension 6384 and asking to make an appointment with a counselor. The Counseling Center is located in Boettcher Health Center, and there are appointment times available throughout the day. Students can call at any time to make an appointment, since Boettcher is open 24 hours a day when school is in session. The first counseling session is free, and there is a nominal fee charged per session after the first. If students are enrolled in the Student Insurance policy, the fees are paid by the insurance. Fee Waivers are also available and no one is refused counseling due to financial hardship.

  Confidentiality
Patient confidentiality is a high priority. As Mental Health Professionals, the counselors at the Counseling Center abide by all state and federal laws regarding client confidentiality.

Laboratory Services

Boettcher Health Center laboratory is fully accredited by COLA (Commission on Office Laboratory Accreditation) and meets all federal government regulations (CLIA) for patient testing. It is staffed by a full-time medical technologist, Monday through Friday and by registered nurses at night and on the weekends.

Some of the tests available include: urinalysis, CBC, mono test, throat culture, rapid strep test, blood glucose, and urine pregnancy test. Specimens for other tests not available at Boettcher are collected and sent via courier to Penrose Hospital Reference laboratory. A contract with Penrose allows Boettcher discounted prices on lab tests. Verbal and written orders from family physicians and other community physicians are honored.

Obtaining Prescription Medications

--Prescriptions dispensed at Boettcher Health Center :

If a student has private insurance (non-CC student insurance): Boettcher Health Center is unable to process pharmacy benefits or bill insurance companies directly. The cost of the medication can be charged to the student’s business account. An insurance claim form will be sent to the student’s Worner box that can then be submitted to the student’s insurance provider for reimbursement, based on plan provisions. Payments from insurance providers sent to Boettcher, will be credited to the student’s business account.
If a student has the Colorado College Student Health Insurance Plan thru UnitedHealthcare:
Prescription drugs are covered at 100%, after a $10 co-pay for generic and a $20 co-pay for brand name, including birth control. The co-pay can be charged to the student’s business account. Exceptions to prescription coverage are addressed in the insurance plan brochure.

--Prescriptions ordered by a provider at Boettcher Health Center

If a requested prescription medication is not available through the Boettcher Health Center pharmacy, the prescription will be ordered through the Medicine Shoppe, unless the student specifically requests a different pharmacy. Health Center staff will fax or email a copy of the student’s insurance information, on file with the Insurance Office, to the Medicine Shoppe. The cost of the medication or insurance co-pay is based on the insurance plan’s provisions. The student is responsible for arranging payment of applicable co-payments or other pharmacy costs.

The Medicine Shoppe is a local pharmacy that accepts most private insurance and is an in-network pharmacy for the student health insurance plan through UnitedHealthcare. The cost of the medication or insurance co-pay is based on insurance plan provisions. The student is responsible for contacting the Medicine Shoppe directly; to fill new prescriptions, to request that current prescriptions be transferred, to provide insurance information, and to arrange for payment. A credit card may be put on file to pay for applicable co-payments or other pharmacy costs. Prescription delivery to Boettcher Health Center is available Monday through Friday and is a special service that the Medicine Shoppe offers at the student’s request. Please allow 48 to 72 hours for prescriptions to be ready for delivery and pick up.

--Prescriptions filled at other pharmacies:

The student is responsible for taking their prescription and their insurance information to the pharmacy of their choice and for arranging for payment to that pharmacy. The cost of the medication or insurance co-pay is based on insurance plan provisions. Students enrolled in the Student Health Insurance Plan through UnitedHealthcare should use a preferred provider pharmacy to receive the maximum prescription benefit. Students enrolled in UnitedHealthcare that use an out-of-network pharmacy will have no covered benefits.

--To Obtain Medication Refills:

From Boettcher Health Center: Please call (719) 389-6384 to request a refill of a medication, dispensed from Boettcher Health Center. Please allow for 48 business hours to have the prescription available for pick-up.
From the Medicine Shoppe: Please call (719) 471-3600 to request a refill. Please allow 48 to 72 hours for prescriptions to be ready for pick up or to be delivered to Boettcher Health Center. There are no prescription deliveries on Saturdays or Sundays.
From All Other Pharmacies: The student is responsible for requesting a refill from their pharmacy of choice. Please provide sufficient time for pharmacies to verify and dispense refills.


Students are responsible for understanding their current insurance policy’s coverage for medications. Review your policy or call the phone number on your insurance card for further information.



Immunization and Travel Clinic

The Health Center offers a wide array of immunizations, as well as providing allergy injections to individuals on a prescribed treatment plan. Immunizations include vaccines for the prevention of flu, chicken pox, meningitis, hepatitis A, hepatitis B, tetanus, Gardasil/HPV, measles/mump/rubella, and screening tests for tuberculosis. The best way to prevent flu is to get a flu shot every fall. Because flu virus strains vary from year to year, a new flu vaccine is recommended each year. Boettcher Health Center offers the injected flu vaccine and we highly recommend this important protection for our students. Immunizations are provided by a RN and appointments may be scheduled during the following clinic times:

            Wed – 1:30 – 5:00 PM
            Fri – 11:30 AM – 3:30 PM

Infirmary

Boettcher Health Center is open 24 hours/day, 7 days a week during the academic year. Students with illnesses or injury, or students recovering from surgery who require specialized care may stay at the health center infirmary throughout the day and overnight. There are 10 beds available in the health center, including an isolation and orthopedic room. Nursing care is available in the infirmary 24 hours a day.
