Fall Starts

Several materials require your prompt attention and reply. Distinct deadlines are listed for each form. Some of the forms require you to include your CC ID number (mailed to you in early June along with your CC username, password, and email address information).

Some forms are online, but others require you to fill them out, print them, sign them, and then scan and email (or fax) them back.

• Certificate of Immunization (Print and have a health care professional fill out)
• Medical Care of a Minor (Only if the student is under 18 years of age)
• PPD (TB) form
