This online form is for currently enrolled Colorado College students who are registered with Disability Services (DS) and have been approved for alternate format services. If you have not been approved for alternate format services and would like to find out if you are eligible for this accommodation, please schedule an appointment with the Director of Disability Services by calling 719-227-8285.

Please submit a separate form for each item you need in alternate format and provide all applicable information, including the correct ISBN.

DS will acknowledge your request for an alternate format within one week of receipt of this online form. At that time, we will let you know the status of your request. When your alternate format is ready for use, DS will notify you and provide information on pick-up and/or delivery options.

Because the process for obtaining alternate formats takes time, we encourage you to plan ahead. The sooner you submit your request, the more likely you are to have your alternate format when it is needed.























By submitting this form, I agree to the following conditions:

  1. I am enrolled in the course listed above.
  2. I will inform Disability Services if I drop the course listed above and/or no longer need the alternate format material.
  3. I understand that I should provide Disability Services with sufficient time to acquire and/or produce alternate formats and that alternate formats are provided on a first-come, first-served basis.
  4. I have purchased, if applicable, the text material listed above and will submit a copy of the receipt to Disability Services upon request.
  5. I understand that the alternate format material provided by Disability Services is solely for my own educational purposes and that, in accordance with copyright law, I will not copy, share, or distribute it. I understand that failure to abide by this agreement may constitute a violation of the Student Code of Conduct and could result in disciplinary action.
  6. I understand that Disability Services will make every effort to provide me with the format I have requested but cannot guarantee that my preferred format will be available. When my preferred format is not available, Disability Services will contact me to discuss alternatives.
  7. I understand that if an alternate format version of a book is not available from the publisher, RFB&D,or other sources, Disability Services may need to scan my book in order to provide me with the alternate format. If this is the case, Disability Services will need to remove the binding from my book, scan it, and have it rebound with a spiral binding.
  8. I will return the alternate format material at the end of the Block for which it is requested or when it is no longer needed for my educational purposes.
  9. I will contact the Disability Services office if I need assistance in requesting alternate formats. (Learning Commons at Tutt Library, Room 152; 719-227-8285)
