The form MUST BE COMPLETED with the Human Resources office BEFORE
you begin working. To do this, you must present specific citizenship
documentation. Please see Human Resources or the Colorado College
Human Resources website at Colorado
College HR Forms for this form and a list of acceptable documentation.
UNTIL THIS FORM IS COMPLETED, YOU ARE NOT ALLOWED TO WORK. NO
EXCEPTIONS.
- W-4 Form (Federal Employee Withholding Allowance
Certificate)
Wages are taxable income and the college is required to have a
completed W-4 form on file for you in order to pay you. For more
information on W-4 forms, refer to www.irs.ustreas.gov.

Q: Where do I get a leave report (EXEMPT STAFF ONLY)?
A: Leave reports are available from your supervisor
and on the HR website at Colorado
College HR Forms. Completed leave reports must be submitted
to your supervisor, who in turn will approve, sign and submit them
to the Payroll Office.
Q: How do I fill out a leave
report?
A: A sample leave report with instructions is at the
HR website Colorado
College HR Forms. Leave reports must be complete and legible
and signed when submitted to supervisors.
Q: When are leave reports
due?
A: Please refer to the Full Time Pay Calendar online
at Colorado
College HR Forms. The report is to be submitted to the Payroll Office
on the last working day of the month, which will always be the same
day as the second payday of the month.
Q: How is my sick and/or
vacation leave accrued?
A: Non-exempt staff earn one sick day per month up
to a maximum of 480 hours (60 days). (These days are pro-rated based
on annual hours.) Vacation accrual is based on your annual hours,
years of service, and whether you are a 12- month exempt or non-exempt
employee. Employees who work less than 12 months do not accrue vacation.
Q: How do I find out what
my leave balances are?
A: Leave balances are found on the bottom of your
paystub. The leave shown is the leave balance as of the end of the
previous month plus the what is accrued in the current month.
Q: What is the process to
document leave I have taken?
A: For exempt staff, any leave taken is entered on the Full-Time Monthly
Leave Report that is to be signed by you and your supervisor and

Q: What happens if I take
more leave time than I have accrued?
A: Your paycheck will reflect pay only for hours accrued
and any remaining unworked hours of your work schedule will be leave
without pay.
Q: What is the number of
personal days I am allowed to take and how is it accrued and tracked?
Full-time non-exempt staff are
allowed three personal days during the college’s fiscal year
(July 1 – June 30). These are deducted from your accrued sick
time. As you take these days, post them on your Full-Time Monthly
Leave Report with the code of “P.”
Q: Can I carryover unused
personal days to the next year?
A: No. Personal days must be taken during the current
fiscal year.
Q: How do I record leave
I have taken?
A: Record hours taken with the appropriate code for
the type of leave you took for each day of the pay cycle on your
Full-Time Monthly Leave Report.
Q: If I don’t take
any leave during the month, do I still have to complete and turn
in a leave report form?
A: Yes. On the Full-Time Monthly Leave Report form,
there is a box to be checked if you have not taken any type of leave
during the month to be reported.
Q: Where do I record overtime
hours worked and when will it be paid?
A: Any overtime hours worked in a week are to be recorded
on the Overtime Report form and submitted to the Payroll Office at
the end of the month along with the Full-Time Leave Report form.
Wages for overtime worked in a month will be paid the first pay
period in the following month.
Q: What happens if I do
not submit my leave report on time?
A: If you (or your supervisor) do
not submit your leave report on time, errors may result in the calculation
of your next paycheck if you took leave without pay. Your leave
balance also may not be correct. Failure to submit your leave report
on time is viewed as a violation of college practices.
