This Capital Project Request Form is for project requests $2,500.00 or greater.  For requests that are less than $2,500.00, please fill out our on line work order request form.  Furniture is not part of the initial Capital Request unless it is part of a larger project.

Time Frame:  Submittal Deadline is March 15th for Next Fiscal Year (FY06 / FY07)
