Prospective Psychology Major,
Before submitting your Psychology major declaration, you must read and
complete this form then submit it to the Department Chair. (Save the
Word document of this page, print
the form, and read and initial each of the items.)
_____ You must have taken, at a minimum, the following courses:
1) PY100 (Introduction to Psychology or FYE Intro. equivalent)
2) PY202 (Research Design in Psychology)
_____ Your advisor should be a Psychology professor.
_____ You have thoroughly read and understood the Psychology Major's Handbook. In particular, you accept the following requirements (initial each):

all course requirements of the major, including courses listed outside
