Please note that when you send the MAT Educational Research Exemption Determination Form to the IRB, you must include your school district letter of approval to carry out research, along with completed copies of any forms you submitted in order to receive that approval. Exemption determination forms without these accompanying documents cannot legally be reviewed for exemption. If you only have hard copies of these forms, please photocopy them and bring them to Armstrong 10 (basement) for delivery to the IRB chair.
