Where can I get the form?
The Campus Activities office can help you complete your Extra-Curricular Record and will input the information into the C.C. computer system.
How can I review my Extra-Curricular Record?
The Campus Activities office will print out an official copy for you. You can also review your record through the Access System.
How do I complete the form?
1. Complete: name, student ID #, date, and phone number
2. Check the organizations/experiences you have had
3. Indicate the year(s) that you participated-please use the calendar years and not the number of years in which you’ve been involved
4. If you are an officer of an organization, then indicate that under the leadership category.
5. Please specify if indicated on the form, i.e., what honor you received.
6. If your involvement is not listed on the form, then please write your involvements and the year(s) in which you were involved in the “Others not listed above” category on the bottom of the back page.
